Health service (re)design: from segmentation to experience based co-design

The intervention cycle

Today the focus is on solution design.
Practice (experiences) and science (research evidence; segmentation, focused factories and
modularization)

The research evidence

Cannot look at whole population for the value, different patient population have different
views of value. Identify patient groups, not just one big general hospital unit.

Segmentation

1

, One can divide patients into segments based on their health/prognosis. It can also be
divided on profiling, delivery of healthcare interventions and resource allocation (do patient
go to the GP, what do they need?). There is a variety of ways to segment patients.
They all segment patients according to their need (demand), or how care which is supplied
can be improved/given (supply). Still don’t know what the best way is to segment patients.

Focus
- Focus can be defined as a way to systematically manage variety by controlling
demand and supply (Peltokorpi et al.)
- Deciding on a focus (on demand, certain patients etc) is important, but then you
limit your variety. You might only focus on one type of disease, but then they do
offer variety in treatment options.
- A hospital can also choose to give a certain type of care to everyone, but then they
are more limited in the treatment options that they can give. Cannot have it all.

Five focus strategies for healthcare

You can provide care based on the population, a group in the population. For demand there
are two types of focus, urgency/severity or illness/symptoms. From the supply focus, a
hospital really looks at what they are good at, what they can deliver best. Don’t focus on
specific patient group, but everyone who is in need of the care/processes that one
performs. The hospital is very good at that process, and they deliver that to everyone.
An outcome based focus looks at care outcomes, promising to get to a certain health
outcome for the patients, e.g. promising to rehabilitate them etc.

KPI? (how many patients see improved results).
