Voor de module 1, EVL 1.2 Versterken van zelfmanagement heb ik een uitgebreid onderlegger geschreven als voorbereiding voor de presentatie die ik heb gegeven. In dit verslag laat ik aan de hand van een casus zien hoe ik het zelfmanagement met behulp van het 5A-model heb kunnen bevorderen.

Kritische beroepssituatie beschreven in leerjaar 4 volgens de Zelcom-model niveau 3. Kan ook gebruikt worden in het derde leerjaar. In deze situatie wordt de zelfmanagement van de patiënt bevordert, waarin de gehele 5A model is uitgewerkt. Beoordeeld met een voldoende. Deze situatie is gebruikt als eindassessment.

Mijn verslag behorend bij module 1.2: Versterken van Zelfmanagement. Beoordeeld met een ruim voldoende, meerdere modellen om zelfmanagement te versterken zijn hierin benoemd, het 5a model is volledig uitgewerkt. Werkgever en cliënt gegevens verwijderd ivm avg, verslag desondanks goed bruikbaar!

Deze KBS gaat over het aanleren van een handeling volgens het 5a-model. De situatie is complex doordat mw. het in eerste instantie zelf niet ziet zitten om te prikken. Later kan ik haar toch overhalen en ondersteunen. Ook heeft ze een lichte taalbarrière.

Voor de leeruitkomst: LU00183 afstuderen 1: beroepsbekwaam verpleegkundig handelen op basis van klinisch redeneren. Ik heb een verslag geschreven aan de hand van een patiëntencasus op een verpleegafdeling binnen een algemeen ziekenhuis. Hierbij is het klinisch redeneren volgens dobber uitgewerkt met twee complexe gezondheidsproblemen. Het 5A-model is uitgewerkt ter bevordering van het zelfmanagement.
