irritation and inflammation of the gallbladder. The presence of gallstones is a significant risk factor because they can directly obstruct the bile duct, leading to infection and
inflammation.
Risk 2: Obesity Individuals who are overweight or obese are at a higher risk of developing gallstones, which are a leading cause of cholecystitis. Obesity increases cholesterol
levels in bile, leading to gallstone formation. It also slows down gallbladder emptying, which further increases the risk of stone formation and inflammation.
Risk 3: Female Gender and Age (Typically over 40) Women, particularly those over 40, are more prone to gallstones and cholecystitis. Pregnancy, hormone replacement
therapy, and birth control pills can increase the risk due to hormonal effects on bile production. Estrogen increases cholesterol concentration in bile, which may contribute to
gallstone formation. Additionally, the risk increases with age as the body’s metabolic processes change.

The gallbladder and pancreas share part of their drainage system, known as the common bile duct. This duct
carries bile from the liver and gallbladder and pancreatic enzymes from the pancreas into the small intestine.
2. Obstruction of the Common Bile Duct
Gallstones that cause cholecystitis can move from the gallbladder into the common bile duct, creating a
blockage.
This blockage can occur at the ampulla of Vater, a small opening where both the bile duct and pancreatic duct
empty into the small intestine.
3. Backup of Digestive Enzymes
When the common bile duct is blocked by gallstones, bile and digestive enzymes, such as amylase and lipase,
can’t flow normally into the small intestine.
This leads to a backup of these digestive enzymes into the pancreas, which causes the pancreas to start digesting
itself (autodigestion), resulting in pancreatitis.
4. Inflammatory Cascade
The obstruction causes inflammation in both the bile duct and the pancreas. This inflammatory response can
escalate quickly, leading to the symptoms of pancreatitis, such as severe abdominal pain, nausea, vomiting, and
fever.
Why It Happens:
Gallstone migration is the most common mechanism linking cholecystitis and pancreatitis. The passage of a
gallstone through the bile ducts can cause transient or prolonged blockage of the common bile duct, impacting
both the bile flow from the gallbladder and the enzyme flow from the pancreas.
This condition is called gallstone pancreatitis, and it often requires urgent treatment to remove the blockage and reduce
inflammation in both the gallbladder and pancreas.
