, Patient is an 18-year-old female who presented in person to our clinic for a follow-up
medication evaluation appointment. We performed a focused exam to assess the efficacy of her
medications on her symptoms of anxiety and depression. Patient has a history of major
depressive disorder and is following up from an initial evaluation after being discharged from an
inpatient hospitalization for five days in June. Patient stated her mood is better and has improved
since the last visit when her Effexor was increased to 187.5mg daily. Patient stated her sleep has
been poor and she feels like she has to ‘force herself to go to sleep’ since stopping Trazadone
several weeks ago. Patient reports sleeping approximately 6-7 hours per night, but sleep is
intermittent, and she wakes up feeling lethargic and not well rested.
Patient stated she was recently hired part-time as a Barista supervisor at a local coffee
shop and credits having previous management experience helping her to obtain this new job.
Patient expressed confidence that she will be able to handle the stress and daily tasks of the
position but is worried that her age may affect how other coworkers and peers view her
credibility since she is so young to be in a supervisor’s role. Patient denied auditory and visual
hallucinations and there were no other indicators that the patient may have experienced episodes
of mania or hypomania. The patient’s mother had a known history of depression and substance
use and died of an overdose approximately six years ago, which is when the patient’s mood
symptoms began. The patient is unaware of her biological father’s medical and mental health
history as she was placed in foster care at two years old and was adopted by her foster parents.
Patient stated she suspects her father is currently in prison.
At the time of our last visit, the patient was prescribed Effexor (Venlafaxine) 187.5mg
daily and denied taking medications for medical problems. Patient is also attending regular
