What is the focused assessment for a patient with pancreatitis? Identify at least 3 body systems and explain what you will physically assess in each system. (ie., cardiac—HR, peripheral pulses, BP, skin color). Be thorough. GI/Abdominal: look for nausea/vomiting, pain (PQRST of pain), inspection-jaundice, blue/gray color of skin, mass, auscultate- bowel sounds or absence of, palpate for mass or any abnormalities, percuss, look for rigidity of stomach, if the patient is bloated, blood in stool, pancreatic enzyme lab values, if patient has had any bloating, gas, diarrhea, or discomfort with food Cardiac: HR, BP, pulses, auscultate heart for abnormalities in rhythm, cardiac lab values, capillary refill, any blue discoloration from lack of blood flow GU/renal: urine-consistency, color, smell, input and output to see if imbalanced, lab values like creatinine and BUN to see renal function, flank pain or tenderness WHY are these body systems your priority? These symptoms/signs correlate with pancreatitis and can help a nurse understand just what parts and systems of the body have been affected by the pancreatitis. GI is a priority because it directly involves the pancreas so symptoms may show up here first. It is important to look at how much of the digestive tract is affected. Cardiac is important because it too can be affected by the pancreas. If cardiac function is affected the body will not be able to meet the metabolic demands and will become further imbalanced.
