NR 451 WEEK 8 DISCUSSION, YOUR FUTURE What does the following statement mean to you? In order to meet the challenges of taking care of older & sicker patients, incorporate patient safety & quality of care into practice, & understand economics & its effect on the workforce, nurses must take a leading role in the future of healthcare. As the largest health care workforce, nurses apply their knowledge, skills, & experience to care for the various & changing needs of patients. A large part of the demands of patient care is centered on the work of nurses. When care falls short of standards, whether because of resource allocation (e.g., workforce shortages & lack of needed medical equipment) or lack of appropriate policies & standards, nurses shoulder much of the responsibility Nurses should take the leading role to partner with patients (and the patient’s family & friends, when appropriate) to realize informed, shared decision making, improve patient knowledge, & inform self-management skills & preventive behaviors. Nurses should encourage the need of working in teams. it will make fewer errors when they work well together, use well-planned & standardized processes, know team members’ & their own responsibilities, & constantly monitor team members’ performance to prevent errors before they could cause harm & also encourage Evidence-based practices should be used in clinical decision making whenever possible & creating a culture of safety. A safety climate is a type of organizational culture & is the result of effective interplay of structure & processes factors & the attitude, perception, & behavior of staff related to safety. Thank you for your discussion board post on your PICO question this week in our discussion board thread. You met the requirements for the PICO & it is nurse driven. Vicki ReplyShow Less Tawana Williams Tawana Williams 3/7/2016 8:35:27 PM RE: PICO Question Modified: 3/8/2016 5:11 AM Profession & class, my PICO question is: How can nurses work together to improve communication with home health care services at time of discharge? I'm currently working as a home health nurse, & all to often patient information is either missing or very limited at the time of start of care (SOC). This can lead to medication administration errors related to missing information, confusion, duplication of services, & poor use of resources for both the patient & the nurse. By providing a type of PIN code at the time of discharge, the healthcare team can access the medical record & have any new & updated information that has changed during hospitalization. Improving continuity of care, reducing possible errors that occur with misinformation, while adhering to patient preference (Dearholt & Dang 2012). P: (Patient, population or problem) Individuals discharged from inpatient hospital admission, to home health services without previous medical history & pertinent information. I: (Intervention) Developing a patient identification number or code (PIN) that would allow follow up services to access the medical record of the individual; in order to provide continuation of care & safety for the patient. C: (Comparison with current practice) Patient discharge summaries, are often vague, limited in content, and/or incomplete. O: (Desired Outcome) Improving the continuity of care from discharge to home health care follow up. By refining the methods of communicating, patient information to the healthcare team post hospitalization. Chamberlain College of Nursing, (2016). The Clinical Question. Week 2 lesson. Downers Grove, IL: Online Publication. Dearholt, S. L., & Dang, D. (2012). Johns Hopkins nursing evidence-based practice: Model and guidelines (2nd ed.). Unfortunately the post that you shared occurs way to often. The ability it share data is certainly a priority, but I also have found that the discharge process itself is not effective in most cases. It seems as if this process is an after thought & is conducted in a hurry, just as the patient is ready to be discharged. It has been my experience that successful releases from a facility to homecare services is accomplished with early intervention & participation. We have learned in our current & past reading assignments that the importance of identifying high risk patients results in positive outcomes & prevents the need for re-admissions. We are all to familiar with the dynamics of homecare & areas that must be assessed to have favorable outcomes include but not limited to identifying a support system, is the home prepared for a discharge for a safety perspective, ad what are the expectations of the patient & family. Medication regime & compliance has always been a challenge for the homecare nurse to manage, particularly when there are multiple prescribers. Thanks for the post, I enjoyed sharing my thoughts with you,
