Chapter 1
Study guide
After completing the reading assignment, you should be able to answer the following questions
in the spaces provided.
1. One of the critical-thinking skills is the ability to identify assumptions. Explain how the
following statement contains an assumption. What is a more appropriate response? “Ellen, you
have to break up with your boyfriend. He is too rough with you. He is no good for you.”
- Assumption: All of Ellen's decision is based on her boyfriend's actions. Based on the
Boyfriend, Ellen can not think for herself.
- Ellen, I am concerned with your well-being. I was hoping we could talk about your
relationship.
2. Another critical-thinking skill involves validation or checking the accuracy and reliability of
data. Describe how you would validate the following data.
Mr. Quinn tells you his weight this morning was 165 lbs.
- Ask Mr. Quinn to step on the scale and determine the right weight based on the records.
The primary counselor tells you Ellen is depressed and angry about being admitted to
residential treatment in the clinic.
- Have a conversation with Ellen and try to understand the reason for her moods. Ask
open-ended. Observe Ellen's mood and behavior while talking with them
When auscultating the heart, you hear a blowing, swooshing sound between the first and
second heart sounds.
- Evaluate again or ask a more experienced individual
The previous RN tells you that Mr. Jones’s family is aloof and does not visit regularly.
- Review the medical records and evaluate their interaction. Gently start a conversation by
asking Mr. Jones' family, and see how they would communicate and the environment of
it.
3. List the barriers to evidence-based practice, both on an individual level and on an
organizational level.
- Organizational level: They are not including time for staff to review the current literature.
Not holding EBP classes for interested Staff and not getting enough training as staff.
- Individual Level: The Nurses lack the research skills to able to evaluate the quality of
research. The Nurses are also isolated from other colleagues knowledgeable in research

, 4. Differentiate subjective data from objective data by placing an S or an O after each of the
following: complaint of sore shoulder _S_; unconscious _O_; blood in urine _O_; family has just
moved to a new area _S_; dizziness _S_ sore throat _S_; earache _S_; weight gain _O_.
5. For the following situations, state the type of data collection you would perform (i.e., complete
database, focused or problem-centered database, follow-up database, emergency database).
OxyContin overdose _Emergency database_; ambulatory, apparently well individual who
presents at outpatient clinic with a rash _Focused_; first visit to a health care provider for a
checkup _Follow Up_; recently placed on antihypertensive medication _Follow Up_.
6. Discuss the impact of racial and cultural diversity in individuals on the U.S. health care
system.
- As the population grows, so does racial and cultural diversity. This affects health care by
creating communication challenges and differences in health needs. Different cultures
may have unique medical requirements, so understanding and respecting these
differences helps patients trust their care and stay healthy.
7. List three health care interactions you have experienced with a person from a culture or
ethnicity different from your own. Were they positive or negative? What could or should have
been done differently?
- Working at Human Resources at Amazon, I have met a lot of diverse cultures and
language barriers. Some spoke little or no English. Although we had a number to call for
translation, I was able to use my phone for translating or try to understand what they
needed without any translation: Positive
- I have a friend who goes to the gym with me. The first time we worked out together, we
used different positions while lifting weights for different muscle groups. We shared our
techniques to improve our workouts. It was positive because we learned new methods
and became gym buddies.
- While I was helping an Associate who spoke a small amount of English with a benefits
issue, I assumed they understood the steps I was explaining. The following day, they
came back frustrated because they hadn't completed the process correctly. This led to a
negative outcome. I should've asked them if they understood or given them a pamplit for
them to follow
8. Using one sentence or group of phrases, how would you describe your own health state to
someone you are meeting for the first time?
- Overall, I’m healthy and active. I go to the gym regularly, I eat a balanced diet, and I take
care of my body mentally and physically
