🦴 ATI Nursing Guide: Mobility Disorders
& Immobility
(Full Clinical Judgment Framework + ATI Exam Triggers)
✅ 1. PRIORITY NURSING INTERVENTIONS (ATI-Style)
🔹 General Immobility (All Clients)
🛌 Reposition every 1–2 hours to prevent pressure injuries
🚶♂️ Encourage early ambulation if possible
●
🧍♀️ Assess skin every shift and use pressure-relieving devices
●
💧 Encourage fluids and high-fiber diet to prevent constipation
●
🫀 Use compression stockings or SCDs for DVT prevention
●
🧠 Perform cognitive stimulation and promote interaction
●
●
🔹 Fractures
❄️ Apply cold packs in first 24–48 hr
🩹 Monitor CMS (circulation, motion, sensation) distal to the injury
●
🛌 Immobilize using splints or traction
●
💊 Administer analgesics, muscle relaxants, and monitor for fat embolism (esp. femur
●
●
fracture)
🔹 Joint Replacements (Hip/Knee Arthroplasty)
🪑 Use abduction pillow (hip) or continuous passive motion (CPM) machine (knee)
🚫 Do NOT flex hip >90° or adduct (for hip)
●
✅ Assist with early mobilization using walker or assistive device
●
💉 Monitor for DVT, PE, wound infection
●
●
🔹 Spinal Cord Injury

, 🛏️ Maintain neutral alignment
🧠 Prevent autonomic dysreflexia (↑ BP, headache, bradycardia)
●
🩻 Log-roll for turning
●
🚻 Implement bowel/bladder programs
●
●
🔹 Parkinson’s Disease
🚶♀️ Promote fall prevention, use assistive devices
🍽️ Encourage small, frequent meals, thickened liquids if dysphagia
●
🕐 Allow extra time for movement, dressing
●
🧠 Encourage cognitive and memory exercises
●
●
📋 2. PLANNING CLIENT CARE (ATI + SMART GOALS)
🔹 Key Goals for Immobility Clients:
● Prevent complications of immobility
● Promote safe mobility or independence
● Maintain skin integrity
● Improve muscle strength and coordination
📌 SMART Goal Examples:
● “Client will ambulate 20 feet with walker and standby assist 3x/day by day 3.”
● “Client will maintain intact skin with no signs of pressure injury throughout stay.”
● “Client will demonstrate use of incentive spirometer 10 times/hr while awake.”
🔹 ATI-Style Planning Tip:
● Select goals that are realistic, measurable, and promote safe, gradual progress
🧠 Exam Tip: Avoid choosing overly ambitious or vague goals like “client will return to normal
mobility in 24 hours”
