This is a case presentation of a 65-year-old Philipino male who presented to the clinic for

medication management after exacerbation of psychotic symptoms after being discharged from

acute inpatient care after a 3-week hospitalization and did not keep taking his medication


Objectives:


1. Colleagues will be able to identify different scales used to access anxiety, depression, and

schizophrenia and use the information to help formulate a treatment plan based on the patient’s

symptoms.


2. Colleagues will be able to identify the DSM-5 criteria for Schizophrenia, bipolar with

psychosis, and brief psychotic disorder


3. Colleagues will be able to discuss options for treating patients experiencing psychosis at

home who refuse hospitalization and do not meet the criteria for inpatient hospitalization as they

do not present as a danger to themselves or others.


Subjective:


CC (chief complaint): “My husband hasn’t been taking his medications for 18 days”.


HPI: The patient is a 65-year-old male presenting for medication management for increased

hallucinations and delusions following his release from inpatient care 18 days ago. His wife is at

his side. She reports he was released 18 days ago following an inpatient hospitalization after

becoming aggressive, ringing the neighbor’s doorbell, and then assaulting them, and having to be

subdued by the police and taken to the emergency room. At that time, he was yelling, appeared

agitated, and delusional about the risk to himself and his wife from the neighbors. He has been
