History of Present Illness (HPI)Max Kings is a 4-year-old Caucasian male

who presents to the clinic accompanied by his father with a complaint

, of involuntary stool leakage into his underwear for the past three

weeks. The father reports that Max has experienced intermittent

abdominal pain for approximately two years, which typically occurs

after eating and resolves within a few minutes. Over the past three

weeks, Max has had watery stools leaking into his underwear 2–3 times

per day, described as streaks of loose stool. The father notes that Max

has a history of constipation, with bowel movements occurring every

4–5 days, characterized by small, hard, pebble-like stools. Max strains

during defecation and reports pain with bowel movements. The father

denies any associated nausea, vomiting, fever, blood in the stool, or

headaches. Max is a picky eater, preferring fried foods and desserts,

drinks three cups of milk daily, and consumes inadequate water. The

family is undergoing a divorce, which may contribute to stress, though

Max’s behavior is reported as otherwise normal. Max is fully potty

trained.Past Medical History
