Patient Case While working in long-term care and rehabilitation facility, I have encountered many patients and residents with multiple co-morbid chronic diseases. For example, an 85-year-old African American male is initially admitted to the facility as a rehabilitation patient after having surgery on a fractured femur. However, his family decides to make him a long-term resident. He has a history of type 2 diabetes, hypertension, and chronic kidney disease. His hypertension is uncontrolled, and he is tachycardic. He takes Metformin for his diabetes and Lisinopril for his hypertension. There are many factors in this case that can affect the pharmacokinetics and pharmacodynamics of his current medications.
