Age: 17 years old
Reason for Visit: Fatigue
History of Present Illness (HPI):
, The patient is a 17-year-old with a complaint of fatigue for approximately [insert duration,
e.g., 3 weeks]. The fatigue is chronic and diffuse and not mitigated by rest or sleep. It
is causing impairment in school performance and sport/activity. Denies recent illness,
fever, weight loss, or night sweats. There are no recent stressors, but
patient does report mild difficulty concentrating and occasional headaches.
Past Medical History (PMH):
There are no chronic illnesses
Immunizations up to date
No history of anemia or thyroid illness

Allergies:
No known drug allergies (NKDA)
Family History:
Mother: Healthy
Father: Hypothyroidism
No family history of chronic fatigue, autoimmune disease, or mental illness
Social History:
Lives with parents
High school student
Denies tobacco, alcohol, or drug use
Sleeps ~6 hours per night (poor quality sleep)
