Care Coordination Presentation to Colleagues
Hello, my name is Megan Shorette. I have been an RN and care coordinator at this
facility for over a year. The foundational concepts of care coordination are the subject of my talk
today. As nurses, we possess substantial power and fulfill a vital role in coordinating patient care
and maintaining their uninterrupted care flow. Before I start, I want to thank everyone
in attendance. This presentation aims to improve your comprehension and skill in care
coordination, aiming to boost patient outcomes and experiences via the direct care services
provided at Garnet Health Medical Center.
Agenda
The purpose of today's session is to investigate the idea of care coordination and to
emphasize the fundamental principles that underlie it. In this session, we will examine effective
, evidence-based cooperation tactics that may be used with patients and family members to
achieve beneficial patient results. In addition, we will investigate the aspects of change
management that directly impact the components of the patient experience, as well as the process
of developing coordinated care plans founded on ethical decision-making. Last but not least, we
will investigate the possible impact that policy measures have on the results and the experience
of patients.
What is Care Coordination?
To facilitate the delivery of health services and achieve the best possible outcome for the
patient, care coordination can be defined as the intentional organization of patient care activities
between multiple health providers, including the patient, the sharing of health information, and
