Remote Collaboration and Evidence-Based Care
Collaboration for Villa Health Remote will be addressed in this presentation. Also,
discrepancies within this facility will be addressed as well as treatment plans for patients and
the advantages and disadvantages of collaboration will be discussed. In order to create a plan,
evidenced based practice will be implemented. After the completion of this presentation, the
reader will have more knowledge and a better understanding of how remote collaboration
ensures that the patient is safe and provided with effective care.
Background
A 65-year-old patient male patient that was diagnosed with CHF presents with
swelling in the lower extremities, fatigue, and SOB. Due to living in a rural area, the patient
has limited access to cardiac specialty care. Past medical history reveals a history of
hypertension. According to the Center of Disease Control, CHF occurs when the heart is
unable to pump enough oxygen and blood to support organs in the body. However, although
heart failure is a serious condition, the heart has not stopped beating. (About Heart Failure,
2024). Congestive heart failure is a result of a weakened or damaged heart muscle, this makes
it challenging for the heart to pump blood effectively. CHF is oftentimes a result of high
blood pressure, DM, heart attack, heart valve disease, and obesity (Abassi et al., 2022).
Patients with congestive heart failure can avoid hospitalization through interventions
and treatment methods. In this patient’s case, challenges arise due to the patient living in a
rural area. During the winter season, traveling is even more of an obstacle due to inclement
weather and daylight savings. Community resources are limited in this particular area
(Manemann et al., 2021). Due to living in such a small town and lack of resources, complex
heart diseases are not usually handled by a primary care physician. Therefore, the patient
should be seen by Cardiology which is not in the immediate area. Factors such as this have a
significant impact on patient care, such as a delay in care.
