The nurse practitioner (NP) certification tests are intimidating for every
examination candidate. In addition, certification examinations take place

What should you expect to find for test question format on the NP boards?
The certifying agencies use a multiple-choice question, the standard testing
instrument used in most high-stakes tests.
The multiple-choice question is more easily approached if you
understand:
■ How it is designed
■ What it is attempting to measure
■ Effective strategies for decoding and answering the question
Work first on increasing and securing your clinical knowledge base prior
to moving to practice questions. When using this book, it is important to
study the didactic content and complete the practice questions.
Multiple-choice questions are made up of a number of parts:
■ Stem (scenario, context, the “story” of the question)
■ Interrogatory (essential question, action that the test taker needs to
consider)
■ Answer choices
Multiple-choice tests do not lend themselves to extraneous detail or
information that does not contribute to the correct answer. Stem scenario or
context is there to support, not confuse, you in your analysis. Typical answer
choices consist of one best answer, one (obviously) wrong answer, and two
partially correct answers. Note that partially correct answers can cause you
to second-guess yourself. Effective test takers have learned to differentiate
between partially correct (the “sometimes” or “yes… but”) and the most
common, best answer.

as a safe entry-level NP, possessing the knowledge base and clinical decision-making

scenario in the test questions. During the certification examination, you
have to decide on the best response given the information in front of you by
applying sound clinical judgment. You will never be given too little
information to make the decision on the correct or best answer. Although
there are a handful of test items that assess factual knowledge, such as
identifying an anatomical landmark or cranial nerve, the majority of test
questions seek to measure higher-order thinking and reasoning skills.

When studying for the family or adult-gerontology NP boards, remember that this practice
is set in primary care, physically distanced from an acute care facility. These are not
subspecialty, acute care, or inpatient examinations.
Domains on the NP Boards
While the NP board agencies use different terminology, there are four major
domains on these examinations: Assessment, Diagnosis, Planning, and
Evaluation. With each question, look at the information in the stem or story
that is presented, and then ask yourself the following:
■ Is this question a test of the ability to perform an appropriate clinical
assessment in a given patient scenario?
■ A question on gathering subjective and/or objective information is one
focused on clinical assessment, which is approximately 25% of board
content.
■ Example: Gathering appropriate health history and physical
examination in a 55-year-old patient presenting with an elevated
blood pressure.
■ Is this question a test of the ability to develop a working diagnosis?
■ A question focused on diagnosis requires the examinee to focus on the
relevant assessment information gathered, then analyze and synthesize
these data. A key principle of differential diagnosis is knowing what
patient groups are at risk for a given condition. In addition, the patient
scenarios will likely be in the form of classic or common disease
