licensure revocation or suspension for a period of time determined by the board

Written and oral examination by the authorized by the Bureau
Limited License - Answer-A person licensed by a reciprocal state to practice the
profession of funeral director shall apply to the Board including:
Original certification from reciprocal state
Fee prescribed
Subject to biennial renewal
Shall become inactive upon the revocation, suspension, placement upon inactive status
or other lapse of holder's license in the reciprocal state. Board may reinstate to active
status upon proof that the holder's license in the reciprocal state is no longer revoked.
Board may take disciplinary action against the holder of a limited license for
unprofessional conduct in the Commonwealth.
Licensure designations - Answer-Each FD license shall designate the name of the
licensee and a place where the licensee may practice
A FD may practice only at an establishment designated in the director's license to
practice funeral directing unless the FD also has a branch office license
Use of name of predecessor - Answer-A sole proprietorship or partnership may be
conducted under the name of a predecessor funeral establishment if the name of the
owner or partners appear as operator on all signs, form and advertising
, Approval of funeral establishment - Answer-Person applying for approval from the
Board to own or operate a funeral establishment, branch funeral establishment,
partnership, or corporation shall submit necessary applications of forms provided.
Must include required fee, statement of applicant of right to occupy premises,
letterhead, statement of funeral goods and svc for at need and preneed, a telephone
listing and an inspection report
Submit withing 30 days of settlement, written notification that prepaid burial contracts
have been transferred
Requirements for funeral establishments - Answer-Preparation room for preparation of
human remains
Proper area or room for reposing of remains
Restroom facilities
Shared physical establishment - Answer-2 or more FDs may conduct separate
businesses at the same physical establishment if each business separately maintains
its own books, records, advertising, signs and all other types and kinds of business
operation, subject to Board approval
Licensees shall give 60 days advance notice of intention to the Board, accompanied by
certified copy of the agreement. Changes shall be reported to the Board
Partnership license - Answer-Partnerships shall be licensed. A license will be issued in
the names of the licensed partners as specific in the agreement. **A copy of the
agreement** shall accompany the application fr the license as well as address of
principal place of business.
