Risk Assessment - in the clinical setting, refers to collection of information about risk
factors during the history, physical and laboratory examination.
Risk factors- - personal characteristics, physiological parameters, symptoms, preclinical
disease states that increase likelihood that individual has or will develop a particular
disease.
Prevalence - number of people with that disease at a given point in time
Incidence - number of new cases of a disease

Explain the rationale for not offering screening laboratory tests - The service causes
harm in the target population ie colonoscopy can cause a perforated colon or use of
aspirin to prevent MI, though causing GI Bleed

Blood Glucose Tests - Three test for screening
• FPG- fasting plasma glucose
• 2-hour post-load plasma glucose- more diagnostic- most accurate
• HbA1c- less sensitive in detecting lower levels of hyperglycemia
• Abnormal fasting glucose 100mg/dL or more random plasma glucose of 200 mg/dL
are considered abnormal
• Need lifetime management, manage DM decrease vascular disease
Human Immunodeficiency Virus - Counsel in HIV testing- awareness alcohol and STI,
abstinence education promoted, condom & spermicide use recommended,
monogamous relationship strongly advised, referral of IV drug users to rehab programs
Syphilis Testing - Test VDRL and RPR- more accurate with later stages
• Penicillin G injections treatment
Prostate Cancer Screening - Abnormal PSA
• Watchful waiting, implantation of radioactive seeds, external beam radiation,
prostatectomy, chemo and or anti-testosterone therapy
Testicular cancer - • Scrotal masses are all potentially abnormal and merit further
investigation
• Ultrasound suspicious lesion, tissue sampling, serum tumor markers
, Pap smears - • USPSTF- screen all women who have ben sexually active and have a
cervix, stop after 65 if normal pap smears in the past
• Test for HPV, ASCUS, CIN, squamous cell carcinoma, adenocarcinoma
Chlamydia - Urine or pelvic specimen
Gonorrhea - • USPSTF- clinicians screen all sexually active women, including pregnant
females and increased, advises against routine screening fo in low risk patients.
Insufficient evidence to recommend high-risk men and low risk pregnant women
• Urine or pelvic specimen
Colo-rectal cancer screening - • >50 y/o family history (1st degree relative, fereditary
nonpolyposis, familial adenomatous poplypopsis) or h/o ulcerative colitis
• USPSTF >50 y/o
• Earlier with higher risk
• Tests: fecal occult blood test, colonoscopy, sigmoidoscopy
Oral cancer screening - • No recommendation but good to do in high risk patients
• Extensive surgery to remove, radiation, chemo, etc.
Skin cancer screening - • Non-melanomas treat with removal, melanomas require re-
excision, biopsy and potentially chemotherapy.
Stages of Change (Transtheorectical model) - o People move through stages when
making lifestyle changes
o Pre-contemplation, contemplation, preparation, action, maintence
Health belief model - o Autonomous - internal motivation and better adherence to
treatment
o Controlled- external forces have more influence
Social cognitive theory (social learning theory) - o States that change and maintenance
are function of an individuals a) outcome expectations and b) efficacy expectations
Identify the four common factors found in psychotherapy research that also differentiate
levels of effectiveness among medical care providers - o Express empathy
o Develop discrepancy- give awareness to the consequences of behavior, recognition of
discrepancy between present behavior and patients own priorities, values and goals
