SUBJECTIVE DATA:
Chief Complaint (CC): Chest pain x3 for the past month
History of Present Illness (HPI): Brian Foster is a 58-year-old Caucasian male who came in for chest pain,
which occurred three times in the past month. He reports that the pain is right in the middle of the
chest, over the heart, and does not radiate. He describes the pain as tight and uncomfortable, with a
pain scale of 5 out of 10. But currently, he is not experiencing any pain. He states that the pain started
earlier this month when he was doing his yard work. The pain has a sudden onset, and is aggravated by
physical activities such as yard work and taking stairs, and relieved by brief rest such as lying still. He
denied taking any medications for the chest pain. According to him, the pain is not related to any food
consumption nor stress. The pain usually lasts a couple of minutes.
Medications: The patient takes Atorvastatin (Lipitor) 20mg PO daily at bedtime for hyperlipidemia,
which he has been taking for a year. The last dose was 10 mg yesterday. He also takes Lisinopril (Prinivil)
20mg PO daily for hypertension and OTC Omega-3 Fish oil 1200mg PO BID. The last dose of fish oil was
8am today.
Allergies: The patient is allergic to codeine with the reactions of nausea and vomiting.
Past Medical History (PMH): The patient has stage II hypertension which he was diagnosed with a year
ago. However, he does not monitor blood pressure at home. He also has hyperlipidemia which he was
diagnosed with a year ago. He has never been hospitalized. He visits his PCP, Dr. Melinda Smith, every
six months, with the last one three months ago for his physical. He has also received EKGs a few times
and an annual stress test with negative results.
Past Surgical History (PSH): He has no past surgical history.
Sexual/Reproductive History: He has been married and monogamous with his wife for 27 years. He
reports being sexually active without the use of contraceptives since his wife is post-menopausal. No
history of STIs nor risky sexual behaviors.
Personal/Social History: He lives with his wife, and his daughter occasionally stays over. He works at a
civil engineering firm in town. He states he has low stress. His hobbies include fishing, sports, small
electronic repairs, and watching his son’s bodybuilding competitions. He has not exercised for a couple
of years. He denies the use of illicit drugs and tobacco products but drinks 2-3 beers every weekend. His
typical diet includes granola bars for breakfast, sandwiches for lunch, and grilled meat and veggies for
dinner. He states he does watch fat and salt intake. He drinks at least 4 glasses of water and a couple of
coffee a day with no soda intake.
Immunization History: Tdap 10/2014, Influenza vaccine this season
