Care Coordination Plan for Stroke Patients
Stroke is the fifth leading cause of death in the United States, accounting for 140,000
American deaths annually. Strokes not only cost the nation $34 billion annually through
healthcare costs, medications, and lost productivity, but also, they are a significant cause of
disability (Boggs, 2022). Patients deal with lifelong deficits, and it is integral for these survivors
to have the necessary support and resources for them to thrive after their hospital stay. A care
coordinator will facilitate effective communication amongst healthcare providers, patients,
families, and caregivers. They will also facilitate linkages between communities and the
healthcare system to address medical, social, and educational needs to achieve that patient’s goal
(Swan, 2019). The use of a care coordinator helps support the multiple needs of the patient and
helps educate the patient about the resources available to them. Patients often don’t know that
there are outpatient programs and financial aid available; educating them can often lessen their
anxiety after discharge. This assessment will explore a care coordinator’s role in assisting
patients that are recovering from a stroke regarding their physical, psychosocial, and cultural
well-being.
Physical Aspect of Coordinating Care for Stroke Patients
The care coordinator should formulate a plan which takes into consideration the stroke
patients' diverse physical needs. This includes ensuring that patients receive proper medical
treatment in addition to rehabilitative services when hospitalized. The nursing care coordinator
may be able to ensure that patients have accessibility to transportation to and from their planned
medical appointments. Furthermore, the nurse care coordinator can guarantee that all patients
have access to the necessary medical supplies and equipment. Depending on the severity of the
