ago with visible 6 inch incision, prostatectomy 2012.

Medications upon admission: Meloxicam 15 mg daily
for 14 days (on day 8), Motrin 600 mg PO PRN OA pain,
Levothyroxine 100 mcg PO Q am.

Social History: Social drinker, 2 pack a day smoker for
40 years, Jehovah’s Witness, widowed with 2 children
and 6 grandchildren, family support system.

❏ Pt will need glasses on during teaching as he
has difficulty seeing.
❏ Pt is hard of hearing, so the tv should be off
or muted during discussion to improve pt’s
focus and ability to hear.
❏ Door should be closed to reduce
environmental noise.
❏ Pt should be sitting up and nurse should be in
a chair at eye level with overhead lights on to
make it easier for pt to read lips to aid in
comprehension.
❏ Medication reconciliation should be printed
out in at least 14 pt font to make it easy for
the pt to view.
❏ Pt should be encouraged to SPEAK UP.
