Modifier 22 - answer-Increased procedural services - When the work required to
provide a service is substantially greater then typically required
Modifier 23 - answer-Unusual Anesthesia - Occasionally, a procedure, which
usually requires either no anesthesia or local anesthesia, because of unusual
circumstances must be done under general anesthesia
Modifier 24 - answer-Unrelated E/M service by the same physician during a post
op period - physician may need to indicate that an evaluation an E/M service was
performed during a post op period for reasons unrelated to the original
procedure
Modifier 25 - answer-Significant, separtately identifieable E/M service by the
same physician on the same day of the procedure or other service - It may be
necessary to indicate that on the day a procedure or service was performed, the
patient's condition required a significant, separately identifiable E/M service
above and beyond the other service provided
Modifier 26 - answer-Professional component - radiology
Modifier 27 - answer-Multiple outpatient hospital E/M encounters on the same
date
Modifier 32 - answer-Mandated services - court ordered
Modifier 47 - answer-Anesthesia by surgeon - anesthesia provided by the
surgeon - does not include local anesthesia
Modifier 50 - answer-Bilateral procedure - performed at the same time
Modifier 51 - answer-Multiple procedures - when multiple procedures are
performed at the same session by the same provider
Modifier 52 - answer-Reduced services - under certain circumstances a service or
procedure is partially reduced or eliminated at the physicians discretion
Modifier 53 - answer-Discontinued procedure - Under certain circumstances the
physician may elect to terminate a surgical or diagnostic procedure - threat of
the well being of the patient
Modifier 54 - answer-Surgical care only
Modifier 55 - answer-Postoperative Management only
Modifier 56 - answer-Preoperative management only
Modifier 57 - answer-Decision for surgury - E/M service that resulted in the initial
decision for surgery
