This agreement ("Agreement") is made on [date], by and between:

RECITALS

 Purpose: The purpose of this Agreement is to define the legal, parental,
and financial arrangements agreed upon by Amber, Susan, and Lori, in
relation to the conception, birth, and parental custody of Lucy, born via
gestational surrogacy.

 Background: Amber and Susan were in a committed relationship and
mutually desired a child to raise together. Amber donated her egg,
which was fertilized by an anonymous sperm donor, and this fertilized
egg was implanted in Lori, who carried the pregnancy to term and gave
birth to a baby girl named Lucy.

TERMS

1. Intent and Parental Rights

 1.1 Amber and Susan, as intended parents, agree that the child, Lucy,
is intended to be their legal child. They intend to co-parent Lucy,
despite any relationship changes between them.

 1.2 Lori agrees that she relinquishes any parental rights over Lucy and
acknowledges that her role was limited to gestation and birth only.

 1.3 Amber, as the biological mother, is acknowledged as the legal
mother of Lucy, with Susan as her intended co-parent.

2. Roles and Responsibilities

 2.1 Lori's role was limited to carrying the pregnancy and undergoing
medical procedures necessary for the successful implantation and birth
of Lucy.

 2.2 Amber and Susan, as intended parents, shall assume full
responsibility for Lucy’s care, custody, and well-being from birth
onwards.
