Hartfalen is een chronische ziekte die vaker voorkomt in oudere mensen. Het kan echter optreden op
alle leeftijden. Hartfalen is een erg dodelijke ziekte, vaak dodelijker dan de meesten soorten kanker.
Bij hartfalen pompt er niet voldoende bloed uit het hart als dat het lichaam nodig heeft. Het hart levert
niet genoeg bloed voor de rest van het lichaam. Oorzaken van hartfalen zijn:
• Coronary artery disease
• Hoge bloeddruk
• Cardiomyopathieën
• Hartklep abnormaliteiten
• Hart aritmieën
• Hartaanval
• Myocarditis
• Congenitale hart ziekten
• Longziekten
• Geblokkeerde arteriën
• Infecties
Het pompen van het hart is onder te verdelen in vier fasen:
1. Diastole (vullen) (a)
2. Isovolumetrische contractie (b)
3. Systole (ejectie) (c)
4. Isovolumetrische relaxatie (d)
In de figuur zijn daarnaast te zien de
eind-diastole (1), opening van de
aortaklep (2), eind-systole (3) en
opening van de mitralisklep (4).
Hartfalen
Systolische disfunctie
Bij systolische disfunctie neemt de helling van de ESPVR af, hij gaat vlakker
lopen. Het eind systolisch volume neemt toe omdat de
pompfunctie/contractiliteit van het hart verminderd. Het eind diastolisch
volume neemt toe door verhoogde preload als gevolg van volume
afname. Er is sprake van hartfalen met reduced ejection fraction
Diastolische disfunctie
Bij diastolische disfunctie neemt de helling van de ESPVR toe, hij gaat
steiler lopen. Het eind systolisch en diastolisch volume neemt af door
minder goede vulling van het hart. Er is sprake van hartfalen met
preserved ejection fraction.
Een gezond hart pompt 60% van het ontvangen bloed in 1 slag weer het
lichaam in, bij hartfalen is dit minder dan 40%. Hartfalen treedt op
wanneer een van beide zijden van het hart de bloedstroom niet kan
bijbenen. Hartfalen kan betrekking hebben op de linker- of
rechterharthelft of beiden. Meestal wordt eerst de linkerharthelft
aangetast.
Systolische dysfunctie treedt vaak op na een myocardinfarct. Het linker ventrikel kan niet meer goed
contraheren waardoor een pompprobleem ontstaat. De ejectiefractie neemt dan af en het eind
