Hazardous Waste Training
1.When is Procurement authorization required?: when the chemicals are diffi-
cult or expensive to dispose of, designated as P-coded once they become waste, highly
toxic, air or water reactive, explosive or flammable liquids over 60 gallons.
2.when is a procurement authorization required for flammable liquids?: 60
gallons
3.what is considered expensive to dispose of?: costs more than 1000 per
container to dispose of
4.What is the form for the specific disposal requirements?: Agreement for Use of
Hazardous Materials
5.what does the Agreement for use of Hazardous Materials cover?: pyrophoric
chemicals, organic peroxides or compounds that can create peroxides and the usual.
6.when should updated inventory control be submitted and in what format?-
: annually and in Excel format.
7.To whom must the chemical inventory be accessible to?: to the whole lab
personnel
8.where is the chemical safety Data Sheet located?: in the room with the chem-
icals and accessible to all people
9.What should be done if your lab does not have a chemical?: check the
JABSOM EHSO database to see if another lab has the chemical

5
, 10.What information must be provided on each chemical?: Chemical name, physical
state, quantity, CAS number, hazard identification (flammable/corrosive/ toxic), and
the storage area.
11.what do Lab inspections and audits cover?: training completion, use of personal
protective equipment, proper use of fume hoods, proper hazardous waste handling and
disposal, Inventory and SDS availability, emergency plan and response equipment
12.What is waste?: a useless by-product of an operation/experiment or any mate- rial
that can no longer be used
13.what is a P-listed waste?: unused acutely hazardous commercial chemical
products
14.what are U-listed waste?: unused commercial chemical products
15.what must waste be to be considered hazardous?: ignitability, corrosive,
reactivity, and toxicity
16.what act required regulations for cradle-to-grave management of haz- ardous
waste?: Resource conservation and recovery act
17.what quantity of waste is the kaka'ako center considered?: conditionally except
