1. INLEIDING

- Kwetsbare ouderen → doen vaker beroep op zorgvoorzieningen
- Smalle definitie: vooral lichamelijke kwetsbaarheid (bv.: bestaan van beperkingen in de activiteiten
van het dagelijkse leven)
- Bredere definitie: de lichamelijke kwetsbaarheid, maar ook de psychische (bv.: depressie) en sociale
kwetsbaarheid (bv.: opleiding & inkomen)

2. SCREENEN EN COMPREHENSIVE GERIATRIC ASSESSMENT (CGA)

- Belganrijke proactieve houding van klinisch redeneren
- Vermijden van cascade breakdown → vroegsignalering!
• Bv.: oudere met delier die valt, de heup breekt, daarna veel op bed ligt en slecht eet en
uiteindelijk blijvend achteruitgaat in de ADL
• Tijdige herkenning delierrisico → preventie maatregelen inzetten
- Werken met de gevalideerde instrumenten tijdens CGA is nodig om te kunnen onderbouwen &
bespreken met de cliënten waarom hij/zij kwetsbaar is + welke preventieve interventies er moeten
afgesproken worden

2.1 STAP 1: SCREENING

Definitie: het in kaart brengen van de risicopopulaties voor een bepaalde aandoening of uitkomst




- Stap 1 = screenen → bij hoogrisicogroepen wordt altijd een vervolg gegeven!
- Screeningsinstrumenten om het risico op slechte uikomsten of de kwetsbaarheid in kaart te brengen
