Het is een samenvatting van de slides van de powerpoint en extra genoteerde items van tijdens de les. De slides waren in het Engels en mijn eigen notities zijn steeds zoveel mogelijk in het Nederlands. Het is dus een mix van de 2 talen doordat het een Engelstalige master is. INTRODUCTION
WHAT AND WHAT IF…

 continue cirkel is verstoord bij neglect (zie fig)

 What: description
- neglect of the contralesional (CL) side, after brain damage
- decreased awareness of the CL space
- decreased attention, exploration and (re)actions on the CL side
- Behavioral: as if the neglected part of space does not exist

 Neglect gebeurt meer na R-side leasion
 Nemen contralesionele zijde niet mee in rekening bij iets uit te
voeren

 What if: negative consequences for
- progress and length of stay in rehabilitation
- mood and quality of life
- activities of daily living (ADL)
 examples: shaving, eating, reading,…
 Zoeken naar een object in een ruimte is moeilijk, ze missen een deel
 Ook lezen kan heel moeilijk zijn omdat ze een deel van de tekst niet
zien
- Imagination exercise
 Spatial neglect is een van de redenen dat ze hun rijbewijs niet terug krijgen na de test

 Niet perse alles op middelijk missen
- Vb: scheren en sommige stukken vergeten

 Neglect kan dalen maar prognostisc niet goed om te hebben
- Indien et maar licht is kan het herstellen
- Indien complexer & ↑ stimuli in omeving= moeilijker

1. WHAT IT IS NOT: DISTINCT FROM
 primary
 hemianopia
 mere attention disorder
 syndroom= veel typen van symptomen

1.1. NOT PRIMARY
 Heilman, 1979:
- “the failure or slowness to report, respond, or orient to novel or meaningful stimuli presented
to the side opposite a brain lesion, when this failure cannot be attributed to either sensory or
motor defect”
- Primaire cortexen leasie
 Progression area’s higher in the brain
 Ze kunnen zien en kunne hun hoofd bewegen naar de hemizijde maar zullen dit niet door omdat ze er
geen aandacht aan geven

 Cf. implicit information processing: contralateral activation in visual cortex, despite unnoticed stimulus
- Visual (Marshall & Halligan, 1988)
- Semantic: priming
 Response time to word in R hemifield faster when related word in L hemifield
is presented first (Ladavas, 1993)

 Iets geven voorafaand aan echt target
 Woord geven aan gezonde zijde= sneller reageren woord geven aan de
aangedane zijde
 Vb: impliciet info verwerken

 Kunnen info van hun zintuigen in een impliciete manier verwerken
- Maar zich er niet van bewust

 Voorbeeld met de twee huizen
- Visual: is er een verschil tussen de huizen?
 Ze geven aan dat er geen verschil te zien is
 Als ze dan een huis moeten kiezen, aan welk huis geven ze hun
voorkeur -> dan kiezen ze het onderste huis
 Impliciet gaan ze het dus wel waarnemen
- Semantic: priming
 cue/prime werd gegeven in hun neglect veld, dit had een relatie met het target
dat gegeven werd aan de gezonde zijde
 Doordat er een relatie is, verminderde de reactie tijd op het target

 Action related
- Glas en fles = logische relatie (boven – onder)
- Indien het object neglect zijde in correcte manier om te gebruiken= sneller kans om
gemerkt te worden
- Indien juiste houden zelfs nog sneller opgemerkt worden indien het met iets lofische
samenhang heeft

1.2. NO HEMIANOPIA
 Neglect gradueler dan hemianopsiee
- P toestand afhankelijk (vermoeidheid) dan hemianopsie
 Neglect= fluctuerender dan hemianopsie

 Hemianopia = half van het visueel veld, ze fixeren zich op een specifieke
plek
 Bij neglect heeft het niet te maken met de visus (hemianopia)




1. monocular vision loss -> lesion optic nerve
2. bitemporal heminopia -> lesion chiasma opticum
3. homonympus hemianopia -> lesion tractus opticus
4. contralateral superior quadrantopia




 De meeste met hemianopia zullen zich naar de linkse zijde
richten
- Oorzaak? Mogelijks ter compensatie
 P met neglect gaat teveel naar contralesionele zijde wijzen
 Heemianopsie= teveel naar aangedane zijde wijzen
1.3. NO LATERALIZED ATTENTION DISORDER (niet gezien)
 No simple lateralized attention disorder without anything more

 Neglect after right hemisphere damage (RHD): usually more frequent, more severe &
more long- lasting
 20-60 % after LHD 30-70 % after RHD
 Reason? -> lateralized functional specialization
 Indien L
- Minder ernstig
- Beter kans herstel

1. LATERALIZED FUNCTIONAL SPECIALIZATION
 Rechter hemisfeer is dominant voor de spatiele
 L-leasie
- Nog iets aandacht voor R-zijde
 R-leasie
- Bijna geen aandacht voor L-zijde




1.1. CF. PHYLOGENETIC TRANSITION
 Lesion in left/ right hemisphere
- Non-human primates
 LHD & RHD: disturbed spatial orienting to CL side
- Humans
 LHD: aphasia, apraxia (disturbed CL orienting to a lesser degree only, subdominant)
 RHD: disturbed spatial orienting to CL side (dominant)
- Doordat onze hemisferen verdeelde taken hebben is da kans op afasie in de LHD
groter en spatial orienting problemen groter in de RHD

 Afhankelijk van waar leasie is of er meer neglect is of niet
