Een volledige en goede samenvatting van probleem 1 voor het vak “Eating, Sex & Other Needs” voor derdejaarsstudenten van de Erasmus Universiteit te Rotterdam. De samenvatting bevat alle verplichte literatuur (DSM, boek, artikelen) en is met afbeeldingen bijgevuld om de stof wat duidelijker te maken. Ik heb zowel het eerste als het tweede jaar van psychologie met een 8 gemiddeld afgesloten, dus mijn samenvattingen werken voor mij altijd erg goed! Voor blok 3.4 heb ik een 7.9 gehaald. Deze sv is na de wg bijgewerkt!

DSM V - Feeding & Eating Disorders
Introductie
Eetstoornissen worden gekarakteriseerd door een persistente verstoring in eten of eet-gerelateerd
gedrag dat resulteert in aangepaste consumptie of absorptie van eten en dat sign. de fysieke
gezondheid of psychosociaal functioneren verslechtert
- De ED’s zijn ​mutually exclusive​; kunnen tijdens een episode niet meerdere diagnoses worden
gesteld (m.u.v. Pica; die kan toegewezen worden in de aanwezigheid van andere ED’s)
- Sommige ED’s komen overeen met substantie gebruik stoornissen, zoals ​craving​ en patronen
van ​compulsief gebruik​ → mogelijk zijn dezelfde neurale systemen betrokken (​regulatory
self-control ​& ​reward​)
- Obesitas wordt niet betrokken als mentale stoornis in de DSM-V, aangezien het niet gezien
wordt als een mentale stoornis. Er zijn echter wel robuuste associaties tussen obesitas en een
aantal mentale stoornissen (BED, depressieve/bipolaire stoornissen, schizofrenie). Medicatie
draagt sterk bij aan de ontwikkeling van obesitas en obesitas kan een risicofactor zijn voor de
ontwikkeling van sommige mentale stoornissen (depressieve stoornissen)

PICA
CRITERIA
A. Persistent​ ​eating​ of ​nonnutrive,​ ​nonfood​ ​substances​ over a period of at least ​1
month
B. The eating of nonnutrive, nonfood substances is ​inappropriate​ to the ​developmental
level of the individual
C. The eating behavior is ​not​ ​part​ of a ​culturally​ supported or ​socially​ normative
practice
D. If the eating behavior occurs in the context of ​another​ mental ​disorder​ (intellectual
disability, autism, schizophrenia) or medical ​condition​, it is sufficiently severe to warrant
additional clinical attention
Specify if:
- In remission​: after full criteria for pica were previously met, the criteria have not been
met for a sustained period of time

DIFFERENTIËLE DIAGNOSE
★ Anorexia nervosa​: pica kan onderscheden worden van het eten van ​nonnutritive​, ​nonfood
substances​. Echter, sommige presentaties van AN bevat dit ook (vb. eten van tissues om
appetite​ te reguleren). Als het eten van dat soort substanties met name wordt gebruikt voor
het controleren van gewicht, dan is AN de primaire diagnose
★ Factitious disorder​: bij deze stoornis kunnen individuen intentioneel vreemde objecten
eten als deel van een patroon van falsificatie van fysieke symptomen. Hier speelt dus een
element van deceptie, die consistent is met willige inductie van schade/ziekte
★ Nonsuicidal self-injury & nonsuicidal self-injury behaviors in personality
disorders​: sommige individuen slikken mogelijk schadelijke objecten door (vb. naalden) in
de context van maladaptieve gedragspatronen geassocieerd met persoonlijkheidsstoornissen
of ​self-injury

RUMINATION DISORDER
CRITERIA
A. Repeated​ ​regurgitation​ (​ oprispingen) ​of food over a period of at least 1​ month​.
Regurgitated food may be re-chewed, re-swallowed of spit out
B. The repeated regurgitation is ​not​ ​attributable​ to an associated gastrointestinal or other
medical ​condition
C. The eating disturbance does ​not​ ​occur​ e
​ xclusively​ during the course of anorexia nervosa,
bulimia nervosa, binge-eating disorder, or avoidant/restrictive food intake disorder
D. If the symptoms occurs in the context of ​another​ mental ​disorder​ (intellectual disability or
another neurodevelopmental disorder), they are sufficiently severe to warrant additional
clinical attention

DIFFERENTIËLE DIAGNOSE
★ Gastrointestinal conditions​: a.d.h.v. fysieke onderzoeken en laboratorium testen moet er
onderscheid gemaakt worden tussen ruminatie stoornis en andere condities gekarakteriseerd
door ​gastroesophageal reflux/vomiting
★ AN & BN​: individuen met AN/BN kunnen ook aan ​regurgitation​ doen met het uitspugen van
eten als bedoeling van het afvoeren (​digesting)​ ingenomen calorieën

AVOIDANT/RESTRICTIVE FOOD INTAKE DISORDER
CRITERIA
A. An ​eating​ or ​feeding​ ​disturbance​ (apparent lack of interest in eating or food; avoidance
based on the sensory characteristics of food; concern about aversive consequences of eating)
as manifested by persistent ​failure​ to meet ​appropriate​ ​nutritional​ and/or ​energy
needs​ associated with ​one​ (​or​ ​more)​ of the following:
1. Significant weight loss (or failure to achieve expected weight gain or faltering growth
in
children)
2. Significant nutritional deficiency
3. Dependence on enteral feeding or oral nutritional supplements
4. Marked interference with psychosocial functioning
B. The disturbance is ​not​ ​better​ ​explained​ by ​lack​ of available ​food​ or by an associated
culturally​ sanctioned ​practice
C. The eating disturbance does ​not​ ​occur​ e ​ xclusively​ during the course of anorexia nervosa
or bulimia nervosa, and there is no evidence of a disturbance in the way in which one’s body
weight or shape is experienced
D. The eating disturbance is ​not​ a ​ ttributable​ to a concurrent medical​ condition​ or not
better explained by another mental​ disorder​. When the eating disturbance occurs in the
context of another condition or disorder, the severity of the eating disturbance exceeds that
routinely associated with the condition or disorder and warrants additional clinical
attention

DIFFERENTIËLE DIAGNOSE
★ Other medical conditions​: restrictie van voedselinname kan voorkomen bij andere
medische condities. Een diagnose voor ​avoidant/restrictive food intake disorder​ vereist dat
de verstoring van inname verder gaat dan die direct worden verklaard door fysieke
symptomen a.g.v. de medische conditie. Het blijft ook bestaan na de medische conditie
★ Specific neurological/neuromuscular, structural or congenital disorders and
conditions associated with feeding difficulties​: voedselmoeilijkheden zijn ​common​ in
een aantal congenitale/neurologische condities. ​Avoidant/restrictive food intake disorder
kan gediagnosticeerd worden zolang alle diagnostische criteria worden bereikt
★ Reactive attachment disorder​: een mate van ​withdrawal​ is een karakteristiek van
reactive attachment disorder​ en kan leiden tot een verstoring in de ouder-kind relatie wat
voeden/voedselinname kan beïnvloeden. De diagnose wordt gegeven als alle criteria worden
bereikt voor beide stoornissen en de voedsel verstoring de primaire focus is
★ Autisme​: tonen vaak rigide eetgedragingen en verhoogde sensorische sensitiviteit. Echter,
vaak leidt dit niet tot de mate van verslechtering die vereist is voor een diagnose voor ARFID
★ Specific phobia, social phobia & other anxiety disorders​: kan lastig zijn om het te
onderscheiden van specifieke fobie (aangezien specifieke fobie kan leiden tot een angst voor
choking​/overgeven, wat kan resulteren in voedselmijding). Wanneer het eetprobleem de
primaire focus wordt, is ARFID de juiste diagnose. In sociale fobie kan het individu bang zijn
om geobserveerd te worden tijdens het eten, wat ook kan voorkomen in ARFID

, ★ AN​: overeenkomst met ARFID dat er restrictie is van energie inname. Echter, bij AN zijn
kernsymptomen ook persistent gedrag dat interfereert met gewichtstoename en verstoringen
in de perceptie en ervaring van het eigen lichaam. Deze kenmerken zijn er niet in ARFID.
Echter, vooral in late jeugd en vroege adolescentie is het lastig om te differentiëren tussen
deze stoornissen. Soms gaat ARFID vooraf aan AN
★ OCD​: kunnen mijding of restrictie van inname vertonen in relatie tot preoccupaties met eten
of geritualiseerd eetgedrag. ARFID kan alleen gediagnosticeerd worden als aan alle criteria
van beide stoornissen wordt voldaan en wanneer het afwijkend eten een groot aspect is
★ MDD​: in MDD kan ​appetite​ verminderd zijn wat leidt tot restrictie van inname. Echter, vaak
lossen deze problemen zich op wanneer de stemmingsproblemen worden opgelost
★ Schizofrenie spectrum stoornissen​: individuen kunnen hier vreemde eetgedragingen
vertonen, mijding/specifieke voedselinname vanwege waanideeën of andere manifestaties van
mijding/restrictieve inname. ARFID kan alleen gediagnosticeerd worden als aan alle criteria
van beide stoornissen wordt voldaan en wanneer de eetverstoring specifieke behandeling
vereist
★ Factitious disorder or factitious disorder imposed on another​: hierbij willen
mensen het doen laten lijken of ze ziek zijn. Ze zouden dan bijv. bepaalde diëten kunnen
volgen of stoppen met bepaald voedsel innemen

ANOREXIA NERVOSA
CRITERIA
A. Restriction​ of ​energy​ ​intake​ relative to requirements, leading to a ​significantly​ ​low
body​ ​weight​ in the context of age, sex, developmental trajectory, and physical health.
Significantly low weight is defined as a weight that is less than minimally normal or, for
children and adolescents, less than minimally expected
B. Intense​ ​fear​ of ​gaining​ w​ eight​ or of ​becoming​ ​fat​, or ​persistent​ ​behavior​ that
interferes with weight gain, even though at a significantly low weight
C. Disturbance​ in the way in which one’s ​body​ ​weight​ or ​shape​ is ​experienced​, undue
influence of body weight or shape on self-evaluation, or persistent ​lack​ of ​recognition​ of
the ​seriousness​ of the current low body weight
Specify whether:
- Restricting type​: during the last 3 months, the individual has not engaged in recurrent
episodes of binge eating or purging behavior (i.e. self-induced vomiting or the misuse of
laxatives, diuretics or enemas). This subtype describes presentations in which weight loss is
accomplished primarily through dieting, fasting and/or excessive exercise
- Binge-eating/purging type​: during the last 3 months, the individual has engaged in
recurrent episodes of binge eating or purging behavior (i.e. self-induced vomiting or the
misuse of laxatives, diuretics or enemas)
Specify if:
- In partial remission​: after full criteria for AN were previously met, Criterion A (low body
weight) has not been met for a sustained period, but either Criterion B (intense fear of
gaining weight or becoming fat or behavior that interferes with weight gain) or Criterion C
(disturbances in self-perception of weight and shape) is still met
- In full remission​: after full criteria for AN were previously met, none of the criteria have
been met for a sustained period of time
Specify current severity:
- Mild​: BMI ≥ 17
- Moderate​: BMI 16-16.99
- Severe​: BMI 15-15.99
- Extreme​: BMI < 15

SUBTYPES
- Het ​subtype binge-eating/purging​ type kunnen zowel ​bingen​ als ​purgen​, maar sommige
doen alleen aan ​purgen
- ‘Crossover’ tussen de subtypes komt weleens voor
