Casus 1De heer/mevr van Ginkel is door de vaatchirurg verwezen naar fysiotherapie i.v.m.
atherosclerotisch vaatlijden in het linker been. Er is sprake van toenemende pijnklachten in
de kuiten bij diverse activiteiten, maar met name bij wandelen. Tijdens het wandelen
beginnen de eerste pijnklachten na ongeveer 150 meter. Na 200 meter moet de patiënt echt
stoppen vanwege ondraaglijke pijn. Deze pijn neemt af na 1 of 2 minuten stilstaan. Hij/zij is
met pensioen en woont alleen.
Hulpvraag
Ik wil graag weer kunnen lopen zonder pijn, of de pijnvrije loopafstand vergroten.
Hypothese
1. Mevrouw van Ginkel heeft d.m.v. atherosclerotisch vaatlijden in het linkerbeen een
verminderde aerobe uithoudingsvermogen (capaciteit).
2. Door een vermindering van lichamelijke activiteit d.m.v. atherosclerotisch vaatlijden
ervaart mevrouw van Ginkel toenemende pijnklachten in de kuit doordat haar
krachtuithoudingsvermogen (KUHV) in de M. Triceps surea is verminderd.
3. Mevrouw van Ginkel kan d.m.v. atherosclerotisch vaatlijden in het linkerbeen een
afstand lopen van 200 meter.
4. Door het hebben van atherosclerotisch vaatlijden staat de kans op risicofactoren of
andere aandoeningen.
Meerdere aandoeningen in de kuit:
1. Mevrouw van Ginkel heeft pijn in de kuiten, omdat ze diabetische neuropathie heeft.
2. Mevrouw van Ginkel heeft pijn in de kuiten, omdat ze varices en chronische veneuze
insufficiëntie heeft.
3. Mevrouw van Ginkel heeft pijn in de kuiten, omdat ze een partiële ruptuur aan de
kuitspier heeft gehad.
4. Mevrouw van Ginkel heeft pin in de kuiten, omdat ze een achterst
compartimentsyndroom heeft.
5. Mevrouw van Ginkel heeft pijn in de kuiten, omdat ze een lumbosacraal radiculair
syndroom van SI heeft.

Stadium 1: ‘’U heeft wel een vernauwing in de arteriën, maar er zijn nog geen
typische klachten van claudicatio intermittens (etalagebenen).

Stadium 2: ‘’U heeft wel vernauwing in de arteriën en er zijn typische klachten
aanwezig van claudicatio intermittens (etalagebenen). Afhankelijk van de afstand die
u kunt lopen, maken artsen een onderscheid tussen 2A en 2B. Bij een stadium van 2A
kunt u pijnvrij meer dan 100 meter lopen. Bij een stadium van 2B treedt de pijn op
voordat u 100 meter gelopen heeft. De meerderheid van de patiënten blijven in
stadium twee. Er is pijn, een moe en stijf gevoel of krampen. Het zit meestal in de
kuit bij het lopen (niet bij het zitten of staan). Het is soms eenzijdig koude gevoel in
de voet.
o Enkel/arm index: > 0,5 en < 0,9
