The patient is a 45 year old African American female. She was brought in by her husband
worried about her excess weight. They report that they are worried that they may never be able to
conceive because of the obesity. They also state that they were advised by their gynecologist to

try working on the patient’s weigh. This is mainly because infertility has been found to be three
times higher among obese women (Silvestris et al., 2018). The patient also reports that her
grandmother was obese most of her life since adulthood, which resulted in diabetes and
hypertension and finally, death. She was worried that she might end up like her grandmother.
During the assessment and evaluation process, the patient and the husband report that she works
remotely at home and spends almost the whole day in front of the computer. She was asked
whether she is involved in any physical exercises and she is stated that she does not exercise. She
notes that she even struggles walking around the house because the weight. The husband also
reports that the patient takes a lot of junk food. She has to take soda every day and loves her
cookies. They also report that they rarely cook healthy food at home and would order pizza
among other unhealthy food. The patient states that healthy food is boring and she always craves
junk food. With such information and the negative impacts, it is important to develop effective
patient centered care plan as will be seen in the concept note.
