AACN ECG M1 L2: ECG Monitoring Systems correctly answered 2023How to prep patient for ECG monitoring 1. Wash the area 2. Clip any hair 3. Abrade (exfoliate) the skin 4. Wipe/Clean area 5. Apply the electrodes 6. Monitor - configure the monitor and alarms. 3-Lead Wire System - Black: L arm - White: R arm - Red: L leg 5-Lead ECG - Black: LA - White: RA - Red: LL - Green: RL - Brown: Any v1-v6 location needed Leads allow you to look at the heart from these 2 planes: Frontal Plane and Horizontal Plane Lead I - Left arm is positive and the right arm is negative - Views lat surface of heart - creates upright waveform deflections on ECG strip - helpful w/ atrial dysrhythmias - helpful to detect changes assoc w/ lateral wall MI Lead II - RA to LL - views inferior surface of heart - used to identify junctional beats/rhythms - used to detect change assoc w/ inferior wall MI Lead III - LA to LL - views inferior surface of heart - detect inferior wall MI Lead aVL (augmented vecotr left) - placed on LA under clavicle - to detect change assoc w/ lateral wall MI aVF lead Uses the left foot electrode as positive and both arm electrodes as a common ground (negative). Horizontal plan lead v1 - unipolar chest lead - views septal wall of LV - useful to monitor ventrical dysrhythmias - brown electrode - goes on R sternal border, 4th IC Horizontal plane lead V2 - unipolar - views LV septal wall - used to monitor ST segment changes - place on L sternal border, 4th IC Lead V3 - unipolar - views LV ant. wall - used to monitor ST-segment changes - place btwn V2 and V4 Lead V4 - views LV lateral wall - used to monitor ST-segment changes Lead V5 Anterior axillary line at the same level as V4 Lead V6 The electrode is at the midaxillary line at the same level as lead V4
