Samenvatting RT periode 4Gynaecologie
Casus Cervix carcinoom
- Vrouw van 53 jaar
- Postmenzopauzaal = niet meer ongesteld in de menopauze
- Postcoïtaal bloedverlies = bloedverlies na de geslachtsgemeenschap
- Diagnose: Cervixcarcinoom T2N0M0
- Tumor breidt zich uit buiten de uterus maar reikt niet tot de bekkenwand of het
onderste derde deel van de vagina.
Behandeling
- Chirurgie
- Radiotherapie
- Chemotherapie
- Bij vaso-invasie, tumordiameter > 4 cm, invasiediepte > 2/3 of > 15 mm wordt er
postoperatieve radiotherapie op het oorspronkelijk tumorgebied en de parametria
overwogen, zodat een betere bekkencontrole wordt bereikt.
- Indien er bij de postoperatieve radiotherapie ook lymfkliermetastasen tot het
doelgebied behoren, kan worden gekozen voor een combinatie met chemotherapie.
(5-6 kuren cisplatine i.v. eens per week tijdens de radiotherapie.)
- Hyperthermie
(kan PET maken voor metastaseringen)
Hyperthermie = hoog temp. (lokale temperatuur ophogen)
- Bij recidieven (=niet alles is eg er zit nog tumorweefsel wat weer verder kan groeien),
indien de patiënt eerder bestraald is
- Bij intolerantie voor chemotherapie (= als bv. de nieren niet goed functioneren, nieren
zijn stuk door de chemo)
- 5 weken achtereen dagelijks RT + 1 of 2 keer per week Hyperthermie één uur 43 ̊.
Hoe? Koud water om patiënt, radiogolven erin. Goed koelen alleen warmte in centrum.
Directe celdood door Hyperthermie liefst zo hoog mogelijk temperatuur dan gaan er meer
cellen dood.
Toegevoegde waarde van hyperthermie:
- Bemoeilijkt reparatie van sublethale schade aan DNA (= DNA beschadigd door RT
maar niet meteen dood, dus je geeft een tikje waardoor hij net wel dood zal gaan,
eiwitten gaan stuk)
- Hypoxische cellen (= cellen waar weinig zuurstof zit) zijn gevoeliger voor
hyperthermie
- Door opwarmen -> toename doorbloeding -> meer zuurstof -> gevoeliger voor RT.
Radiotherapie:
- Uitwendig
- Brachytherapie = radioactieve bronnen inbrengen
