WG SyncopePoll: welke wegraking is een syncope?
1. Door drukte
2. Door epilepsie
3. Door hypoglycemie
Goede antwoord = drukte. Andere zijn niet syncope. Syncope = voorbijgaand
bewustzijnsverlies door cerebrale hypoperfusie.
Wegraking is overkoepelende term
Vallen gaat alleen maar over onverwacht terecht komen op lagere positie zonder
bewustzijnsverlies.
Reflexsyncope
- Vasovagaal = door pijn, angst, staan, drukte
- Situationeel = door mictie, hoesten, defaecatie, uitrekken
- Sinus-caroticus = autonome activatie (zweten, bleek, misselijk)
Overlap vallen en synope bij ouderen. 50% van de wegrakingen weet de oudere niet dat er
sprake was van bewustzijnsverlies. Oudere denkt dus dat ze gevallen zijn, terwijl er wel
degelijk sprake was van syncope. Presyncope bij minder ernstige afwijking (lichtere
hypoperfusie). De anoxia reserve tijd is 5-6 seconde voordat je wegraakt.
Wat is de incidentie?
Incidentie is 2-6% per jaar. 1/3 recidief binnen 2 jaar. 40% heeft licht-ernstig letsel. Met de
leeftijd neemt de kans op letsel toe. De meest voorkomende oorzaak bij ouderen is
orthostatische hypotensie (15-30%), bij jongeren de vasovagale collaps. Sinus caroticus 20%.
