Week 1 summary: Diabetes Mellitus
Our case study on John was challenging, as he is sedentary, obese and has hyperlipidemia. Without a doubt,
lifestyle modifications are necessary in conjunction with pharmacotherapy. Behavioral interventions have a
goal of reducing weight by at least 7% and increasing physical activity to a minimum of 150min/week.
Metformin is the first-line agent for the prevention of diabetes and should be initiated in the prediabetes
stage. As John’s healthcare provider, it is your goal to properly assess him to prevent any micro- &
macrovascular complications while ensuring he maintains a high quality of life. Clinical guidelines suggest this
is best achieved by actively engaging the patient in their own healthcare and implementing “Patient-Centered
Collaborative Care.” In most adults, providers suggest a glycemic A1c target of < 7%; however, there are going
to be situations where a more stringent target of <6.5% or a more lenient target of <8% are appropriate as
well.
The 2021 ADA clinical guidelines also stress monitoring a patient’s mental health, as many diabetics are
susceptible to Diabetic Distress. This is a distinct diagnosis that is directly related to a patient’s emotional
response with the burden of managing their disease state. Based on the descriptors presented in the case
study, John is at risk and should be assessed further.
