Week 1
Provide one specific example of how you achieved the weekly objectives.
What did you learn from this week to challenge your beliefs or attitudes?
How will this knowledge improve your effectiveness as a practice scholar?
This week I found that there were care guidelines specifically for geriatrics although some of
them are not updated frequently, and the CPG are good resources to help me to identify
problems to correct them if needed. I also reviewed the roles of the stakeholders that are
needed to make changes. Using these skills will assist me in making changes (evidence-based)
in skilled/long-term care and to assist me in translating evidence into practice.
Week 2
Provide one specific example of how you achieved the weekly objectives.
Why is the information important?
In what ways will you use this learning?
I did not realize how much diabetes cost to care for individuals. I knew it was expensive, but
wow. Illinois has a diabetic plan to help decrease diabetes. Who knew? It does not matter if you
have diabetes or not you are a stakeholder as it relates to healthcare costs. In LTC, diabetes
patients have had it diagnosed for a while.
Week 3
Provide one specific example of how you achieved the weekly
objectives.
What is the benefit of the learning?
What values can you reaffirm or want to reconsider after this
learning?
This week I looked at diabetes on a local level. I also, for me, reviewed how research studies
protect people and the ethical standards and responsibilities that a researcher must maintain.
This week the learning helps to remind of the ethical responsibilities that I have to maintain and
can help me translate research to get the desired patient outcomes. I want to ensure that
patients come first and they remain protected during research and being able to see that the
research I am reviewing that this was done.
Week 4
Provide one specific example of how you achieved the weekly objectives.
Which change model or conceptual framework did you find most valuable and
why?
In what ways, if any, do your practices inhibit reform?
