Patient Information: M.S., 44, Female, Caucasian, None provided S. CC burning on urination HPI: Reports burning with urination and feeling very itchy and uncomfortable in that area. Onset: 2 days. Location: Pelvic area, vulvar area. Duration: Constant. Characteristics: Vulvar itching and burning with urination. Aggravating Factors: None reported. Relieving Factors: None reported. Treatment: Drinking more water, Monistat cream and cranberry pills; “nothing really has seemed to work.” Severity: No pain scale reported. Seems to feel “not right.” Current Medications: Takes a daily multivitamin and a B complex vitamin. Allergies: NKDA. PMHx: No chronic illness or injuries. Recently treated for strep throat with a 10 day course of penicillin. Completed 1 day ago. Hospitalized x 2 for childbirth, no surgeries
