A. Nursing Sensitive Indicators
Nursing sensitive indicators reflect the structure, process and outcomes of nursing care.
The structure of nursing care is indicated by nursing staff levels, staffing mix, educational level
and experience level. Process indicators measure methods of nursing assessment, methods of
care, types of interventions and staff satisfaction. Outcomes are directly relatable to quantity and
quality of nursing care [Sau13]. In the provided scenario had the nurses providing care for Mr. J.
understood and identified the nursing sensitive indicators, hospital acquired prevalence of
pressure ulcers and prevalence of restraints they may have avoided the adverse outcomes
experienced by Mr. J and could have improved his outcomes. By understanding hospital acquired
pressure ulcers the staff could have done a comprehensive skin assessment on Mr. J using either
a Braden Scale or Norton Scale to assess Mr. J’s risk of developing a pressure ulcer and taken
appropriate measures to prevent their development such as using a special mattress, utilizing a
patient turning schedule or padding patient bony prominences. And once it was noted a pressure
ulcer was developing the nursing staff could have taken appropriate interventions to halt the
worsening of the ulcer such as not putting patient back in bed on spine. By understanding
nursing sensitive indicator prevalence of restraints the nursing staff would have recognized that
in Mr. J case restraints seem unnecessary and tried other less restrictive measures such as a
bed/chair alarm, utilizing a UAP as a patient sitter, or moving Mr. J closer to nursing station to
allow for more frequent checks and reorientation if necessary. Nursing care has a direct impact
on specific patient outcomes and that is why knowing and understanding nursing sensitive
indicators are important [Ame07].
