Service Line for Mobile Diabetes Management Clinic
Proposed Nursing Service Idea
The proposed nursing service line is designed to implement a comprehensive diabetes
management clinic with the primary objective of supporting individuals living with diabetes and
mitigating the risk of long-term complications. This initiative will be nurse-led, prioritizing
patient education, community-based awareness campaigns, preventive care interventions, and
the systematic monitoring of blood glucose and hemoglobin A1C levels through specimen
collection. Advanced Practice Registered Nurses (APRNs) will serve a pivotal role in this model,
as their prescriptive authority and capacity to order diagnostic testing will facilitate timely and
effective disease management. Considering the significant health consequences associated with
diabetes—including cardiovascular disease, neuropathy, stroke, retinopathy, renal failure,
lower-limb amputations, and premature mortality—the establishment of such a program is of
critical importance. The success of this service line will also be contingent upon robust
leadership. Essential competencies such as effective communication, the ability to inspire and
influence, and the capacity to motivate interdisciplinary teams will be indispensable in fostering
collaboration, reducing hospital admissions, improving clinical outcomes, and enhancing
community health engagement (American Diabetes Association, 2025).
Importance
Diabetes continues to represent a significant public health challenge and remains one of
the leading causes of mortality in the United States. Current estimates from the Centers for
Disease Control and Prevention (CDC) indicate that over 37 million Americans are affected by

diabetes, including both diagnosed and undiagnosed cases (Centers for Disease Control and
Prevention, 2024). Within Washington County, Wisconsin, the prevalence of diabetes has
increased by 9.1% since 2013, signaling a troubling upward trajectory. The county also reports a
premature mortality rate of 8,600 deaths, exceeding the national benchmark of 7,300.
Furthermore, 16% of Washington County residents self-report their health status as fair or
poor, compared to 12% of individuals nationwide. Epidemiological data additionally highlight
elevated diabetes prevalence among adult males and members of the Hispanic population in
the region (County Health Rankings & Roadmaps, n.d.).
The proposed service plan is directed toward adults between the ages of 18 and 65
residing in Washington County, a group disproportionately affected by diabetes-related health
burdens. At the state level, approximately 3.2 million adults—representing 10.5% of
Wisconsin’s population—have received a formal diagnosis, while an estimated 884,400 remain
undiagnosed and are consequently at heightened risk for adverse outcomes. The economic
impact is equally considerable. According to the American Diabetes Association, individuals
with diabetes generate healthcare expenditures more than twice as high (2.3 times greater) as
those without the condition, underscoring the urgent need for targeted intervention and
prevention strategies (American Diabetes Association, 2025).
