, MANAGEMENT PLAN TEMPLATE

PROBLEM STATEMENT:

This patient is a 66 year old female with a past medical history of osteoporosis, osteoarthritis, and chronic back

pain. She has a FMH of breast cancer, HTN, HLD, and OA. She reports 1 day of sudden, constant back pain,

rating it a 9/10 lumbothoracic non radiating pain after lifting a heavy garden soil bag. She denies any associated

symptoms. Physical exam reveals stable vital signs, kyphotic posture, limited ROM. Upon palpitation, she

presents with pain and localized tenderness to the L1 bone prominence.

PRIMARY DIAGNOSIS:

Fracture of lumbar vertebra (S32.0).

Fracture of the vertebra, or spinal bones, can occur in the older adult population. Osteoporosis is the

leading risk factor for obaining a vertebra fracture. Other risk factors include trauma, cancer, infection, long

term steroid use, and hyperthyroidism that all lead to weakened bones (Whitney, 2023). Symptoms include

back pain at or near the fracture level, worsening pain with movement, radiating pain, and nerve pain (Olinger

et al., 2025). In this patient’s case, the diagnosis of a vertebral fracture is supported by both clinical

presentation and diagnostic testing. The patient reported a constant, sudden onset of severe back pain after

heavy lifting. On physical examination, she presented with kyphotic posture, limited ROM, and pain over the

L1 bony prominence. Additional imaging is required to confirm the suspected diagnosis.
