Renal #1 Major Points of Content:

 Fluid and Electrolytes
 Acute Kidney Injury
 Chronic Renal Insufficiency
 Glomerulonephritis
 Pyelonephritis-Urosepsis
 Nephrotoxic Medications
 Diuretics

, Fluid & Electrolytes
Amount and Composition of fluids

 Body fluid compartments

 ICF (inside cells)
 ⅔ of body fluid is in ICF
 ECF (Outside cells)
 ⅓ of body fluid is in ECF
 Divides into intravascular (blood), interstitial, and transcellular fluid spaces
 Intravascular (fluid within blood vessels): contains
plasma, the effective circulating volume. Approximately
3 L of the average 6 L of blood volume in adults is made
up of plasma. The remaining 3 L is made up of the blood
cells: erythrocytes, leukocytes, and thrombocytes
(platelets).
 Interstitial space: contains fluid that surrounds the cell
and totals about 11-12 L in an adult. Contains lymph
 Transcellular space: smallest division of ECF and contains 1 L
 Cerebrospinal, pericardial, synovial,
intraocular, pleural fluids, sweat, and digestive
secretions
 Body fluid moves between the ECF and ICF to maintain equilibrium between the spaces
 Semipermeable membrane surrounding each cell allows
certain fluids and electrolytes to move between the spaces
 Loss of fluid disrupts equilibrium -> body wants to maintain homeostasis
 If too much moves from ICF to ECF -> cellular dehydration
 If too much moves from ECF to ICF -> cell swelling
 Third space fluid shift:
 Fluid that is not lost from the body but unavailable for use by ICF or
ECF -> loss of ECF into space that does not contribute to equilibrium
between ICF anf ECF
 Accumulates in membrane bound spaces in body (peritoneal cavity, pleural space)
-> ex: ascites, pleural effusion, pericardial effusion, angioedema
 Signs of 3rd spacing, intravascular FVD
 Decrease in UO despite adequate fluid intake
 Decreases b/c fluid shifts out of intravascular space ->
kidneys receive less blood and attempt to compensate
by decreasing UO
 Increased HR, decreased BP, decreased CVP, edema,
increased body weight, I&O imbalances
