C-SSRS (Columbia-Suicide Severity Rating Scale):
What it is: A standardized suicide risk screening instrument designed to assess both suicidal
ideation (thoughts) and suicidal behavior (actions).
How it works: It uses plain-language "yes" or "no" questions. The standard "screener" version
relies on a minimum of 2 and a maximum of 6 questions depending on what the patient endorses:
1. Wish to be dead
2. Non-specific suicidal thoughts
3. Thoughts of method (without plan/intent)
4. Active suicidal intent (without a specific plan)
5. Intent with a specific plan
6. Suicidal behavior (asks if they have ever prepared or attempted suicide, or had an
interrupted/aborted attempt).
Clinical Purpose: It measures the severity and immediacy of risk to help clinicians safely triage
care without over-referring individuals.
