NUR245 Lecture Week 5 Day 1: Chronic
Obstructive Pulmonary Disease, Oxygen
Therapy
COPD: chronic airflow limitation
*** preventable, treatable, progressive disease of airflow limitation which l/t
respiratory failure
Chronic inflammatory response in airway/lungs primary caused by:
- cigarette smoking (#1 cause) or noxious particles of gas
EDUCATION: stop smoking
- monitor for COPD exacerbations and/or coexisting illnesses (asthma) that
will exacerbate the disease
*** COPD = example of hypercapnic respiratory failure
only one we need to know
COPD: not the same as chronic bronchitis or emphysema
Chronic bronchitis: chronic cough + sputum for at least 3 months in 2 years
Emphysema: destruction of alveoli without fibrosis
stop smoking = #1 COPD prevention
Risk Factors include:
- cigarette smoking (airway obstruction)
>40 years old with smoking history of 10 years with >10 cigarettes/day =
may have possible COPD diagnosis
- frequent respiratory illness (in childhood), HIV, TB
- asthma (COPD-overlap syndrome) = pathologic overlap between the two;
older adults may have components of both
- air pollution (inflame airways which leads to remodeling/lung destruction/
pulmonary vascular changes)
- aging (smoking + aging = bad)
- more common (men)
- women who smoke = 50% more likely to develop COPD
- *** genetics ***
AATD = COPD risk factor; on the exam!
