SECTION ONE: QUESTIONS 1–200
Question 1
A 58-year-old male with a 20-year history of type 2 diabetes presents for routine
follow-up. His most recent HbA1c is 8.9%. He is currently taking metformin 1000
mg twice daily and glipizide 10 mg daily. He has a BMI of 34 kg/m² and an eGFR of
52 mL/min/1.73m². Which medication is most appropriate to add to his regimen?
A. Insulin glargine
B. Empagliflozin
C. Liraglutide
D. Pioglitazone

RATIONALE: For patients with type 2 diabetes and established cardiovascular
disease or high cardiovascular risk, GLP-1 receptor agonists such as liraglutide are
preferred due to their cardiovascular benefits and weight loss effects.
Empagliflozin is also an option but the eGFR of 52 is acceptable; however,
liraglutide is more appropriate given the BMI and cardiovascular risk profile.
Pioglitazone causes weight gain and fluid retention. Insulin is typically reserved
for patients with severe hyperglycemia or when oral agents fail.
Question 2
A 34-year-old female presents with a 3-day history of dysuria, frequency, and
urgency. She is 28 weeks pregnant. Which antibiotic is most appropriate?

,A. Nitrofurantoin
B. Trimethoprim-sulfamethoxazole
C. Cephalexin
D. Ciprofloxacin

RATIONALE: Cephalexin is safe in pregnancy and effective for uncomplicated
urinary tract infections. Nitrofurantoin is acceptable in the second trimester but
should be avoided near term due to risk of hemolytic anemia in the neonate.
Trimethoprim-sulfamethoxazole is avoided in the first trimester and near term.
Fluoroquinolones are contraindicated in pregnancy.
Question 3
A 72-year-old male presents with progressive dyspnea on exertion and bilateral
lower extremity edema. Echocardiogram reveals an ejection fraction of 35%.
Which medication has been shown to reduce mortality in heart failure with
reduced ejection fraction?
A. Furosemide
B. Amlodipine
C. Sacubitril-valsartan
D. Digoxin

RATIONALE: Sacubitril-valsartan (Entresto) is a combination ARNI that has
been shown to reduce mortality and hospitalizations in heart failure with reduced
ejection fraction compared to enalapril. Furosemide provides symptomatic relief
but does not reduce mortality. Amlodipine is not indicated for HFrEF. Digoxin
reduces hospitalizations but not mortality.
Question 4
A 45-year-old female presents with a 6-month history of fatigue, weight gain, cold
intolerance, and constipation. Laboratory results show TSH 12.5 mIU/L and free
T4 0.4 ng/dL. Which medication is first-line?
A. Levothyroxine
B. Liothyronine

, C. Levothyroxine
D. Desiccated thyroid

RATIONALE: Levothyroxine is the standard of care for primary
hypothyroidism. It is converted to T3 in peripheral tissues and has a long half-life,
providing stable levels. Liothyronine is not first-line due to its short half-life and
risk of fluctuating levels. Desiccated thyroid is not recommended due to
inconsistent hormone content.
Question 5
A 28-year-old female presents with a 2-day history of severe unilateral headache,
nausea, and photophobia. She has a history of similar episodes. Which medication
is most appropriate for acute treatment?
A. Acetaminophen
B. Ibuprofen
C. Sumatriptan
D. Propranolol

RATIONALE: Sumatriptan is a triptan that is effective for acute migraine
treatment. It works by stimulating serotonin receptors to cause vasoconstriction
and reduce inflammation. Acetaminophen and ibuprofen are less effective.
Propranolol is used for migraine prophylaxis.
Question 6
A 62-year-old male presents with a 3-day history of productive cough, fever, and
pleuritic chest pain. Chest radiograph reveals a right lower lobe infiltrate. Which
antibiotic is most appropriate for outpatient management?
A. Amoxicillin
B. Azithromycin
C. Levofloxacin
D. Doxycycline

RATIONALE: For community-acquired pneumonia in adults without
comorbidities, macrolides (azithromycin) or doxycycline are first-line. For patients
with comorbidities or recent antibiotic use, respiratory fluoroquinolones
