Ecchymosis to chest wall and lower abdomen secondary to seat belt. Neurologic
assessment and abdominal assessments are negative. No lacerations. Abrasion to
left and center forehead.

Received 21-year-old driver involved in motor vehicle accident via ambulance to
emergency department (ED). Client lost control of vehicle due to wet road
conditions. Vehicle rolled two times down steep embankment prior to landing
upside down; emergency medical service (EMS) extracted client from vehicle.
Ecchymosis to chest wall secondary to seatbelt. Loss of consciousness for
,approximately 5 minutes with residual amnesia. Does not remember the accident.
