I believe that most healthcare providers, doctors and nurses, and other medical professionals that offer any services run their organizations under the fee-for-service process. This way when a patient comes in or a service or a visit, they pay for that service, test, test, visit. Another factor is the purchasing and updating of technological equipment. This also explains why the spending in healthcare is so high.
