Dear Mr Thandanani,
I am writing to bring to your attention concerns regarding the service and repairs carried
out on our panel van at Flashycars Workshop on 22 September 2025.
The vehicle was booked for servicing and for the repair of its rear brake lights. Although
Mr Thakhani indicated that he would inspect the vehicle and report back to us, this did
not happen. When the vehicle was used for deliveries the following day, we discovered
that the plugs had not been replaced and that the rear brake lights were still not

1. TERMS OF REFERENCE
On 1 September 2025, Ms Sangweni, Chief Executive Officer (CEO) of The Big-Three
Safari Lodge, commissioned the secretary of the lodge to investigate the possible
factors responsible for the delay in completing the planned refurbishment of the lodge.
The investigation was prompted by the report that only 40% of the refurbishment work
had been completed, despite the scheduled completion date of 31 October 2025. The
purpose of the investigation was to establish the factors contributing to the delay and to
provide recommendations for addressing the situation.
The completed report and recommendations are to be submitted to Ms Sangweni by 30
September 2025.
2. PROCEDURES
2.1 Review of the refurbishment schedule
