1. Cardiovascular System & Perfusion
Heart Failure (HF) Management Protocols
Exam questions heavily differentiate between Left-Sided and Right-Sided failure.
Clinical Manifestations: Left vs. Right
Left-Sided Heart Failure (Lungs):
• Dyspnea, orthopnea, and paroxysmal nocturnal dyspnea (PND).
• Crackles on lung auscultation (fluid backing up into pulmonary bed).
• Frothy, pink-tinged sputum (late sign of pulmonary edema).
Right-Sided Heart Failure (Systemic):
• Jugular venous distention (JVD) at 45-degree bed elevation.
• Dependent peripheral edema (ankles, sacrum).
• Hepatomegaly (liver engorgement) and ascites.
CRITICAL PRIORITY: Pulmonary Edema
If a Heart Failure patient suddenly develops severe dyspnea, tachycardia, and pink frothy
sputum, they are in acute pulmonary edema. Immediate Nursing Interventions
(UNLOAD FAST):
1. Positioning: High-Fowler’s with legs dependent (decreases preload).
2. Oxygenation: Administer high-flow O2 (non-rebreather).
3. Pharmacology: Administer IV Diuretics (Furosemide) immediately.
Myocardial Infarction (MI) & Ischemia
Tissue death due to sustained ischemia. The mantra for immediate suspected MI treatment
is MONA (Morphine, Oxygen, Nitroglycerin, Aspirin) — though clinical priority dictates
administering Oxygen and Aspirin first.
