How to use this guide: NAPLEX calculation questions are free points if you know the setup. Cover the right column, write the formula from memory, then
check. Units matter as much as numbers — always confirm mg vs mcg and lb vs kg before you calculate.
1 Renal & Body Size

Cockcroft-Gault CrCl CrCl = [(140 − age) × weight (kg)] ÷ (72 × SCr); × 0.85 if Use IBW if obese, actual weight if underweight, AdjBW if
female dosing weight-based drugs in obesity.
IBW Male: 50 + 2.3 × (inches over 60) Memorize both constants — 50 and 45.5.
Female: 45.5 + 2.3 × (inches over 60)
Adjusted body weight AdjBW = IBW + 0.4 × (TBW − IBW) Use for aminoglycosides and other weight-based dosing in
obese patients.
BMI weight (kg) ÷ height (m)2 ≥30 = obese; drives which weight to use above.
