made me come in today." The patient also expressed distress about feeling under surveillance
by individuals outside his window. He alleges that he can both hear their presence and observe
their shadows.
HPI: ST I, a 53-year-old male, sought clinical assistance due to persistent concerns about being
harassed and watched by unidentified individuals. His sister encouraged him to seek help.
According to ST, he perceives the presence of these individuals and can see them, despite their
apparent lack of awareness that he can do so. ST maintained that he had been living without
causing any disturbance, but he felt persistently targeted by certain individuals who frequently
gathered outside his window, where they watched him.
ST believes that these individuals are acting on government orders to surveil him, resulting in
exorbitant taxes that he claims "reach the sky." He reported encountering and hearing these
, individuals for several weeks. Additionally, during his sessions with healthcare providers, ST
mentioned seeing a bird and hearing heavy metal music in the doctor's office.
ST used to reside with his mother, but she passed away three years ago. Currently, he lives alone
but harbors concerns that his sister is collaborating with the government to interfere with his
life.
ST faces persistent sleep disturbances due to the loud voices he hears, leading to frequent
wakefulness. Regarding his substance use history, ST disclosed regular alcohol consumption and
smoking three packs of cigarettes daily. He also admitted to prior marijuana use but
discontinued it after his mother's passing.
ST denied using cocaine, engaging in binge eating after drug or alcohol use, experiencing
blackouts, or having seizures. He was previously prescribed various medications, including
Seroquel, Thorazine, Haldol, and Risperidone, but has refused to take them.
ST's family history indicates his paternal grandfather had paranoid schizophrenia, while his
