2. Core Renal Pathophysiology
GFR and renal function
Glomerular filtration rate (GFR) is a central estimate of how effectively the kidneys filter plasma. It is generally more
informative about overall filtering function than a single urine-output measurement. Serum creatinine and other markers are
used clinically to estimate kidney function, but the exam concept is to recognize GFR as the key functional measure.

RAAS sequence
Renin is released by the kidney when renal perfusion or effective circulating volume falls. Renin initiates conversion of
angiotensinogen to angiotensin I; ACE then converts angiotensin I to angiotensin II. Angiotensin II promotes vasoconstriction
and stimulates aldosterone release. Aldosterone increases sodium reabsorption and promotes potassium secretion in the
distal nephron. Water follows retained sodium, supporting extracellular volume.

Memory chain: Renin → Ang I → Ang II → Aldosterone → Na■ retained → water follows.

ADH
Antidiuretic hormone is released from the posterior pituitary and increases water reabsorption in the collecting ducts. More
ADH generally means more concentrated urine and less free-water loss.

Aging kidney
Normal aging can involve reduced renal blood flow, fewer functioning nephrons, and reduced renal reserve. Do not confuse
normal aging with abrupt renal failure.

Nephrotic syndrome
Nephrotic syndrome is characterized by heavy protein loss in urine, hypoalbuminemia, edema, and often hyperlipidemia. In
children, periorbital edema can be an early recognizable manifestation.
