1. History of Present Illness (HPI) – Detailed Narrative

Katherine Moore is a 66-year-old widowed White female who presents with a painful rash that
began approximately 4–5 days ago. She describes the onset as a burning, tingling, and “electric-
shock” sensation on the right side of her mid-back and wrapping around to the right anterior
chest/abdomen in a band-like distribution. Within 24–48 hours she developed clusters of small,
fluid-filled blisters on an erythematous base in the same dermatomal distribution
(approximately T6–T8 on the right).

The pain is severe (8–9/10), constant, burning and stabbing in quality, worse at night and with
light touch of clothing (allodynia). She rates the pain as “the worst I’ve ever felt.” She has had
low-grade fever (99.6–100.4 °F), mild malaise, and decreased appetite for the past 2–3 days.
She denies cough, shortness of breath, chest pain, abdominal pain, nausea, vomiting, diarrhea,
dysuria, headache, visual changes, or neurological deficits elsewhere.

She has not started any new medications, soaps, detergents, or topical products. She has not
been hiking, camping, or exposed to poison ivy/oak. No known insect bites. She reports a
history of chickenpox as a child. She has never had shingles before. She is up to date on most
adult immunizations but has never received the recombinant zoster vaccine (Shingrix).
